New Patient Registration

We look forward to your first visit as a new patient with Oaks Medical Center. If you would like to make the check-in process even faster, fill out the registration paperwork ahead of time. You can either submit your completed forms via email or you can print them out and bring them with you to your visit.

Remember to bring along a valid drivers license and your current insurance information. If you are currently taking prescription medication, please bring a list of those medications, including dosage and frequency.

If you are being sent by your company for any of our corporate services, including drug screens, breath alcohol testing, physicals, or work-related injuries, fill out the Authorization Form, have it signed by a supervisor, and bring it with you to your visit.

If you have HMO insurance coverage and one of our physicians is not listed on your insurance as your primary care provider (PCP), you will need to contact your insurance company and change your PCP to any of the Oaks Medical Center providers. Once you make the change to your insurance, any of our primary care providers can see you as a patient.

All payments are due at the time of service with the exception of those items that are submitted to your insurance provider. Once the contracted rates are verified with your insurance carrier, your co-pay amount is due prior to your visit with one of our medical providers. If you do not have insurance, payment for your visit or any additional procedures must be received in advance of those activites.

Payment for services at Oaks Medical Center may be in cash, by check, or with Visa, Master Card, Discover, or American Express. We cannot accept temporary or out-of-state checks.
