USCIS Immigration Physicals

Oaks Medical Center is staffed with a Civil Surgeon selected by the Department of Homeland Security approved to perform immigration physicals. All USCIS immigration physicals are performed by appointment only. Several of our staff, including our Civil Surgeon, speak Spanish. If you are unable to speak English or Spanish, you may need to bring your own translator.

You can save some time by completing the USCIS paperwork ahead of time. You can either submit your completed forms via email or you can print them out and bring them with you to your visit.

Please remember to bring all of your supporting documentation with you to your appointment:

• Picture ID - Passport AND Drivers License (if available)
• Marriage Certificate - Required for females that want the paperwork in their married name
• Immunization Records - If records are not in English, they MUST be translated prior to your visit
